Obtaining a Targeta Sanitària Individual (CatSalut)Official step
  1. CatSalut states the application is made in person at the assigned CAP (Centre d'Atenció Primària) using a TSI request form.
  2. The published requirement lists an identity document: DNI, NIE or passport.
  3. The published requirement lists a municipal registration certificate (certificat d'empadronament) dated less than three months before application, or authorisation for CatSalut to verify it with other administrations.
  4. Where the applicant is a Social Security affiliate, the published requirement lists a photocopy of the affiliation document.
  5. CatSalut states the card is mailed free of charge to the applicant's home address once processed.
