- The Comunidad de Madrid lists a certificado de empadronamiento, issued within 90 days, proving registration with the ayuntamiento at the applicant's address.
- The published requirement lists identity and residency documentation: DNI/NIE and valid residency authorisation.
- Applications are submitted digitally via Cuenta Digital with recognised electronic signature, or in person at an authorised registry office.
- The published eligibility condition is municipal registration (empadronamiento) plus recognised healthcare entitlement via the INSS.
- The published process states the card is mailed to the assigned centro de salud approximately 3–4 weeks after application, and that care can be accessed before the physical card arrives.
